There are six steps within the application you will need to complete:

1. Fill in the required information marked by an asterisk (*).

2. Navigate to the second section of this page, beneath “Please select if any of these apply”. If none apply, select “None”. If applying for the MWPA grants, you will need to select one or more of the access or functional needs in order to continue the application.

3. Click “next” to continue.
